Our Care Coordinators support patients both directly and indirectly throughout their care journey.
They work closely with GPs and other members of the practice team to support specific groups of patients within our surgeries, including people living with long-term conditions.
Their role may include helping patients and their carers access the right care, support, and services at the right time. Care Coordinators may also support patients living in care homes by helping to coordinate their care and improve communication between healthcare professionals, ensuring a more joined-up approach to their healthcare.
Each of our surgeries uses Care Coordinators in ways that best meet the needs of its own patient population. This means their responsibilities may vary slightly between practices, reflecting local priorities and the needs of the patients they support.
