Our Clinical Pharmacists work either within our surgeries or remotely as part of the practice team.
They work closely with GPs and other healthcare professionals to support the safe and effective use of medicines. Their role includes carrying out structured medication reviews for patients taking regular medication, helping to ensure medicines remain appropriate, effective, and safe.
Clinical Pharmacists can also provide advice on medicines, support patients with long-term conditions, answer medication-related queries, and help optimise prescribing to improve patient care. They may also work with patients following a hospital stay to ensure any changes to their medication are managed safely.
