You can now self-refer to the Skin Lesion/Mole Clinic Diagnostic Hub if you are over 18 years old.

If you can answer ‘YES’ to each of the following statements, please click on the link below to book your appointment.

1. I only have 1-2 Skin Lesion(s)/Mole(s) I would like to be checked.
2. My Skin Lesion/Mole is NOT located on my genitalia.
3. My Skin Lesion/Mole is NOT larger than a £1 coin (23mm).
4. I have NOT received previous treatment for this Skin Lesion/Mole.
5. My Skin Lesion/Mole is NOT open and/or wet, and without bandages.
6. My Skin Lesion/Mole can be clearly seen and is NOT covered by hair/tattoo.
7. I am able to travel to one of three GPPC locations to the Skin Lesion/Mole Clinic Diagnostic Hub below:

• Fryatt Hospital, 419 Main Road, Harwich, CO12 4EX.
• Clacton and District Hospital, Tower Road, Clacton, CO15 1LH.
• Colchester Primary Care Centre, Turner Road, Colchester, CO4 5JR

If you answer ‘No’ to any of the above questions please contact your individual GP practice stating which reason you are unable to attend the Diagnostic Clinic.
