Social prescribing link workers

Our Social Prescribing Link Workers help patients access the right support to improve their health and wellbeing. They work with people whose needs may be affecting their health but cannot always be addressed through medical treatment alone.

Patients may be referred to a Social Prescribing Link Worker for a wide range of reasons, including living with long-term health conditions, experiencing low-level mental health concerns, feeling lonely or socially isolated, or facing complex social or practical challenges.

Link Workers take the time to understand what matters most to you and work with you to identify the support that best meets your needs. This may include connecting you with local community groups, voluntary organisations, welfare and benefits advice, exercise or wellbeing programmes, befriending services, carers’ support, housing advice, or other community resources.

By helping people access practical, emotional, and social support, Social Prescribing Link Workers aim to improve wellbeing, build confidence and independence, and help people take greater control of their health. They work closely with GPs and the wider healthcare team to ensure patients receive the most appropriate support for their individual circumstances.
